At Hunter Care Group, our Coordination of Supports team (pictured) is dedicated to making a difference by guiding individuals through their NDIS journey and helping them achieve meaningful outcomes.
Daniel’s story is a fantastic example of what can be achieved with the right support and a focus on building trust. With the compassionate guidance and assistance of his Coordinator, Malina, Daniel gradually began engaging with therapies and supports that aligned with his needs and goals.
Fast forward to today, and Daniel has achieved so much in just under a year:
Engages fortnightly with his OT and psychologist. Attends weekly appointments with his drug and alcohol counsellor. Has been abstinent from substances for over six months, actively implementing daily strategies suggested by his therapists. Participates in a group art class every Thursday, where he is thriving creatively. Works one day per week through his DES provider, with plans to increase his hours soon. Had his driver’s licence reinstated after being medically signed off as fit to drive, a significant milestone he worked hard to achieve.It’s been an inspiring journey for Daniel, and we’re honoured to have been part of his story. Stories like Daniel’s highlight the impact of consistent support, tailored services, and strong collaboration between participants, therapists, and providers.
