Today we’re celebrating a huge milestone for Paul, a younger person living in residential aged care due to his diagnosis of MS. Paul is wheelchair and bed bound, and until now, he hadn’t accessed the community in several years. ♿💚
There were many barriers holding Paul back from getting out, including anxiety and the physical challenges of leaving his environment safely. That all started to change when Paul began working with his new HCG Coordinator of Supports, Sophie. 👥✨
Sophie collaborated closely with Paul’s fantastic Allied Health team, the aged care facility, and his Community Access Provider. After months of planning, training and trials, the team found the right setup to support Paul in achieving his goal. ✅
That goal was to have lunch at Hexham Bowling Club. Today, he made it happen. The smile on his face said it all. 😄🍽️
These moments are what it’s all about. Congratulations Paul, and thank you to everyone who helped make this day a reality. 🙌
