Our NDIS support coordination team is one of the largest in the Hunter region. From this pool of specialists, our managers match the participant with a COS with the right skills and experience to best support and advise you in utilising your NDIS plan, in line with your needs.

How an NDIS coordinator of supports can help

From application to execution, NDIS journeys bring all kinds of decisions, big and small. Your coordinator of supports will be by your side every step of the way to help you make an informed decision. We can:

  • Assist in understanding what supports are funded in your NDIS plan and how we can implement them together
  • Work with you to develop a comprehensive goal plan using a SMART goal template to ensure you’re at the centre of all agreed goals
  • Collaborate with all NDIS support workers and services involved in your care. This may include Your GP, Health, TAFE, the Department of Housing and other NDIS services

  • Set up your NDIS portal access on the myGov website

Support options

A support coordinator will present you with all appropriate supports and service providers for your circumstances. Always respecting your choice, we will make sure you retain full control of your NDIS plan.

Problem resolution

If you have concerns about a service provider at any stage, your COS will help you to resolve the issue. We can act as a mediator if needed, to resolve issues with current providers and if needed, use our knowledge and network to find another provider that meets your standards.

Plan maintenance

Once your NDIS plan is up and running, we will help you keep track of its impact and make amendments wherever possible. With ongoing reflection and revisions, we can make sure your support continues in line with your funded and personal goals.

Frequently asked questions

Support coordination, also known as coordination of supports, helps you to navigate the supports under your NDIS plan as efficiently and cost-effectively as possible. It involves prioritising and planning suitable supports depending on your unique needs.

This includes pinpointing and connecting you with the right providers, as well as maintaining and adjusting resources as your needs evolve. That way, you can build your capacity to eventually manage these supports independently.

Three levels of support coordination can be included in an NDIS plan. Depending on your needs and circumstances, you may receive:

Level 1: Support connection

Suitable for NDIS participants with lower-level care needs, this short-term support coordination will help you understand your plan, connect with service providers and get the most out of your funding. It should also boost your confidence and ability to manage the NDIS plan on your own.

Level 2: Coordination of supports

Coordination of supports empowers you to develop the skills required to understand and implement your NDIS plan. This includes helping you to identify and connect with the right supports. Our goal is to help you achieve your goals, such as maintaining relationships, managing day-to-day tasks, leading a more independent life and taking part in your community.

Level 3: Specialist support coordination

Specialist support coordination is reserved for NDIS participants with complex needs that demand additional assistance. A highly-trained specialist support coordinator will help you access service providers, manage supports and overcome barriers to ensure consistent service delivery.

A support coordinator plays a key role in the management and implementation of your NDIS plan. Assisting with various aspects throughout your NDIS journey, we will explain your budget and offer advice on how it may be used to achieve your goals. We can also assist you through any concerns with support providers and communicate these issues with the NDIS.

Ultimately, our goal is to ensure your NDIS plan is set up and maintained properly so that you can build capacity. Depending on the level of support coordination you need, your aims and circumstances, a coordinator of supports can:

  • Help you to create a plan for your particular circumstances and coordinate your supports effectively;
  • Collaborate with family members, formal service providers and other supports to make sure your needs are fulfilled;
  • Establish service agreements with suitable providers, flag any obstacles to access and keep track of supports to ensure they remain suitable as your needs change; and
  • Help you to strengthen your skills to manage your NDIS plan. For example, we’ll assist you with preparing for reassessments and reviews.

Your NDIS support coordinator should:

  • Have proven experience and qualifications in disability care;
  • A genuine commitment to supporting you in reaching your goals;
  • A deep understanding of the NDIS, always adhering to its Code of Conduct;
  • Understand your objectives, adapt to your needs and focus on the supports that will work best for you.
  • Empower you to feel in control in all areas of your life, providing compassionate support as you work towards your goals.
  • Help you to access supports that make sense for your needs, and develop the capacities needed to live independently; and
  • Adopt a highly flexible approach that helps you to navigate and overcome unexpected changes or challenges.
